Speak with Thorben at J-10 in Batallia Downs.
Next, head into the Eldieme Necropolis(s), also at J-10 in Batallia Downs [S]. Check the sarcophagus at I-9 to receive the key item Right Map Piece. Then, head to H-8 and check the gravestone to get the key item Middle Map Piece. Last, talk to Erik at H-7 for the final key item, Left Map Piece. He is there when all four of the gates -- Leviathan's Gate, Odin's Gate, Shiva's Gate and Titan's Gate -- are open. He is also there when some combinations of the gates are opened and closed.
